If you have any of the following labor disputes with your employer, and either you or your employer is unwilling to mediate, the mediation has failed, or a mediation agreement has been reached but not fulfilled, you may apply for labor dispute arbitration at a competent labor and personnel dispute arbitration committee in Beijing.
1. Dispute regarding the confirmation of an employment relationship;
2.Dispute arising from the conclusion, fulfillment, modification, termination, or dissolution of a labor contract;
3.Dispute related to dismissal, discharge, resignation, or departure;
4.Dispute concerning working hours, rest and leave, social insurance, welfare, training, or labor protection;
5.Dispute over wage, medical expense(s) for work-related injury(-ies), economic compensation, or severance pay;
6.Any other labor dispute as stipulated by laws and regulations

Notes
1. Criteria: The case must meet the service scope defined by the Law of the People's Republic of China on Mediation and Arbitration of Labor Disputes and the Rules for Handling Labor and Personnel Disputes Arbitration Cases (Order No. 33 of the Ministry of Human Resources and Social Security). Specifically:
- The dispute falls within the legally defined scope of labor and personnel arbitration.
- There is a clear arbitration request and factual basis.
- The applicant is a natural person, legal entity, or other organization that has a direct interest in the case, and the respondent is clearly identified.
- The required application materials are complete.
According to jurisdictional regulations, the applicant must submit a written application to the municipal or district-level labor and personnel dispute arbitration committee.
2. The arbitration application form must include the following information: (I) The laborer's full name, gender, date of birth, identity document number, residence address, postal address, and contact number; the name, office address, postal address, and contact number of the employer, as well as the name and position of the legal representative or the main person in charge; (II) The arbitration requests and facts and reasons supporting the requests; (III) Evidence and its sources, including any witnesses' names and addresses.
